Overview: The recognition program of Shands Jacksonville Medical Center, Inc., dba UF Health
Jacksonville, is a service of UF Health Jacksonville. This program is provided for all
eligible employees of UF Health Jacksonville. All users of this website agree that their
access to and use of this website are subject to the terms and conditions set forth herein
and all applicable laws. Any access is undertaken at the user’s own risk. These terms and
conditions may change without prior notice.
Eligibility: Eligibility for the UF Health Jacksonville Recognition Program, Applause, is limited to
current UF Health Jacksonville Company employees in good standing. Points earned in
the program cease to exist when this eligibility is severed. No credit, transfer, or
redemption of Reward Points is available after eligibility is severed.
Changes and Disputes: UF Health Jacksonville reserves the right, without prior notice, to discontinue or change
the program at any time.
UF Health Jacksonville reserves the right to change these terms and conditions at any time.
All questions or disputes regarding eligibility for this program, the eligibility of rewards,
the availability of rewards or points, the terms and conditions of the program will be
resolved by UF Health Jacksonville at its sole discretion.
Participant Enrollment: Each eligible employee is automatically enrolled in the UF Health Recognition Program,
Applause, and will use their employee ID number to gain access to the UF Health
Jacksonville Reward website. Upon initial login, participants may create their username
and personal password.
Earning Rewards: The earning and accumulation of reward points begins on an eligible participant’s date of
hire. Eligible participants may earn points through the Standing Ovation recognition
program. The jobsite points will be posted quarterly, the month following the end of each
quarter. Eligible participants may also earn additional points throughout the year for other
administrative sponsored initiatives and contests. These points will be distributed at the
sole discretion of UF Health Jacksonville management.
Redemption and Deduction of Points:
- Any Reward Points not redeemed before termination or employee initiated separation will expire.
- Reward Points may only be used to redeem merchandise from the online catalog. Points are not redeemable for cash.
- The Reward Points you accrue do not constitute your property. You can not transfer Reward Points to another employee. Reward Points have no monetary value except to the extent specifically provided for by a reward.
Claiming a Reward:
- UF Health Jacksonville reserves the right to modify merchandise available in the Reward Points On-Line Catalog at any time and without notice.
- When you have obtained sufficient points, you may redeem an item(s) from those described in the On-Line Catalog.
- Specific merchandise is subject to availability and substitutions may be made based on availability or market conditions.
Statements of UF Health Jacksonville Reward Points Earned: This website provides eligible participants with details of transactions, as well as UF Health Jacksonville Reward Point balances.
Termination of Program: UF Health Jacksonville reserves the right to terminate the Recognition Program, Applause, at any time without prior notice to you. If the program is suspended or terminated, any unredeemed Reward Points must be redeemed within one month of the date of termination of the program.
Participant Program Tracking: It is the responsibility of each eligible participant to use their awarded points to redeem items form the UF Health Jacksonville Reward Points Online Catalog. Awarded points can only be redeemed for merchandise presented in the Reward Points Online Catalog.
Promotional Events: From time to time, UF Health Jacksonville, at its discretion, may sponsor events intended to promote participation in the Recognition Program, Applause. These events may be terminated at any time, without cause or warning.
Taxes: Merchandise redeemed under the program is subject to applicable federal, state and/or local taxes. Payment of taxes resulting from these awards is the sole responsibility of the participating employee and not UF Health Jacksonville or any of its affiliates or agents. UF Health Jacksonville will report earnings under this program in accordance with Federal, state and local regulations.
Shipping and Handling: Merchandise redeemed under the program is subject to shipping and handling charges. The shipping and handling charges are included in the point value associated with each item in the On-Line Catalog and are represented in the overall value of the item.
Privacy Notice: This website is housed and operated by Online-Rewards on behalf of UF Health Jacksonville. The information collected on this website will be used by UF Health Jacksonville in connection with the UF Health Jacksonville Applause Recognition Program.
